ABSTRACT

Introducción. La circunferencia de cintura (CC) es indicador de obesidad abdominal y riesgo cardiovascular en adultos. En Perú, la obesidad ha aumentado a diferente magnitud por área de residencia y poco se sabe de la influencia del consumo de alimentos ultraprocesados (AUP) sobre este fenómeno en población adulta. Objetivo. Evaluar la asociación entre ingesta de AUP y circunferencia de cintura en adultos peruanos por área de residencia. Materiales y métodos. Estudio transversal de datos secundarios de 745 adultos con información de ingesta dietaria (un recordatorio de 24 horas) de la Encuesta Nacional Vigilancia Alimentaria y Nutricional por Etapas de Vida 2017-2018. Los AUP fueron caracterizados según la clasificación NOVA. La ingesta AUP como el porcentaje relativo de la ingesta energética total (%), dividida en terciles. La CC medida como punto medio entre última costilla y cresta iliaca. Se usó regresión lineal múltiple ponderada y análisis estratificado según área de residencia. Resultados. La edad promedio fue 37,2 años. La ingesta de AUP promedio fue 14,7% (IC95%: 14,2 ­ 15,3). Comparado con adultos en el menor tercil de ingesta de AUP, aquellos en tercil medio tuvieron mayor CC ( 0,73; IC95%: 0,22 ­ 1,24; valor p= 0,007). Al estratificar por área de residencia, adultos rurales del tercil medio tuvieron mayor CC en comparación con primer tercil ( : 1,85; IC95%: 1,17 ­ 2,53, valor p < 0,001). Conclusiones. En adultos peruanos, la ingesta de AUP se asoció a CC en áreas rurales, aunque no de forma lineal. Más estudios son necesarios para entender la naturaleza de esta asociación(AU)


Introduction. Waist circumference (WC) is an abdominal obesity and cardiovascular risk indicator among adults. In Peru, obesity prevalence has been increasing unequally between residence areas, and the influence of ultra- processed food (UPF) consumption on WC in Peruvian adults remains unclear. Objective. Evaluate the association between UPF consumption and waist circumference by residence setting among Peruvian adults. Materials and methods. A cross-sectional secondary analysis of dietary intake data (single 24-hour recall) from 745 adults aged 18 and 59 years old from the "Vigilancia Alimentaria y Nutricional por Etapas de Vida 2017-2018" National Surveys was performed. The NOVA system was used to characterize the UPFs, and the exposure was the percentage of total energy consumed from UPF per day (%), in quantiles. WC (cm) was assessed at the middle point between the last rib and the iliac crest. Weighted linear regression analysis stratified by residence areas were conducted. Results. The mean age was 37.2 years. The mean percent of total energy consumed from UPF was 14.7% (95%CI: 14.2 ­ 15.3). Those in the middle tertile of UPF consumption, had higher WC ( 0.73; 95%CI: 0.22 ­ 1.24; p-value = 0.007) compared with those in the lower tertile. In the stratified analysis, those in the second tertile in rural areas have more WC compared with the first tertile ( 1.85; 95%CI: 1.17 ­ 2.53, p-value< 0.001). Conclusions. In rural areas, UPF consumption was associated with waist circumference, but it does not follow a linear association. Further studies are needed to understand the rationale behind these results(AU)

ABSTRACT

Objective: To analyze the agreement between body mass index (BMI) and waist-to-height Ratio (WHtR) to identify preschool and school children with cardiovascular risk factors (CRFs). Methods: Three-hundred-twenty-one kids were divided into preschool (3-5 years) and school children (6-10 years). BMI was used to classify children as overweight or obese. Abdominal obesity was defined with a WHtR ≥0.50. Fasting blood lipids, glucose and insulin were measured, and the homeostasis model assessment of insulin resistance (HOMA-IR) was calculated. The presence of CRFs and multiple non-waist circumference (non-WC) metabolic syndrome factors (MetS-Factors) [high HOMA-IR, high triglycerides and low high-density lipoprotein cholesterol (HDL-C)] were analyzed. Results: One-hundred-twelve preschool and 209 school children were evaluated. WHtR ≥0.50 classified abdominal obesity in more than half of the preschool children, exceeding those classified with overweight+obesity by BMI (59.5% vs. 9.8%; p0.05). There were similar proportions of school children classified with abdominal obesity by the WHtR and overweight+obesity by the BMI (18.7% vs. 24.9%; p>0.05). There was substantial agreement between WHtR and BMI to identify school children with high total cholesterol values, low-density lipoprotein cholesterol (LDL-C), triglycerides, non-HDL-C, insulin, HOMA-IR, low HDL-C values, and the presence of multiple non-WC MetS-Factors (kappa: 0.616 to 0.857, p<0.001). Conclusion: In preschool children WHtR ≥0.5 disagree with BMI results, but in school kids, it has good agreement with the BMI to classify the children´s nutritional status and to identify those with CRFs.


Objetivo: Analizar el grado de acuerdo entre el índice de masa corporal (IMC) y la relación/cintura estatura (rCE) para identificar niños con factores de riesgo cardiovascular (FRC). Métodos: Estudio transversal analítico con 112 niños preescolares (3-5 años) y 209 escolares (6-10 años). El sobrepeso y la obesidad se clasificaron con el IMC y la obesidad abdominal con la rCE ≥0.50. Se tomaron muestras sanguíneas en ayuno para análisis de glucosa, insulina, lípidos y cálculo del índice HOMA-IR. Se analizó la presencia de FRC y de múltiples factores del síndrome metabólico (Factores-SinMet) diferentes a la cintura [HOMA-IR elevado, triglicéridos elevados y colesterol de alta densidad (HDL-C) bajo]. Resultados: rCE ≥0.50 clasificó con obesidad abdominal a más de la mitad de los niños preescolares, excediendo el número de niños clasificados con sobrepeso+obesidad por IMC (59.5% vs 9.8%; p0.05). Fueron similares las proporciones de niños escolares clasificados con obesidad abdominal por la rCE o con sobrepeso+obesidad por el IMC (18.7% vs. 24.9%; p>0.05). Hubo acuerdo sustancial entre la rCE y el IMC para identificar niños escolares con valores elevados de colesterol total, colesterol de baja densidad, triglicéridos, colesterol no-HDL, insulina, HOMA-IR, valores bajos de HDL-C y la presencia de múltiples Factores-SinMet diferentes a la cintura (kappa: 0.616 a 0.857, p<0.001). Conclusión: En niños preescolares la aplicación de rCE ≥0.5 no presenta acuerdo con el IMC, pero en escolares presenta un acuerdo sustancial con el IMC en la clasificación del estado nutricional y en la identificación de niños con FRC.

Abstract BACKGROUND: Obesity is a risk factor for falls in older adults, but the effects of body fat distribution and its interaction with other factors are not well established. OBJECTIVES: To verify the occurrence of falls among older adults with and without abdominal obesity and the effects of sociodemographic, health, and behavioral variables on this outcome. DESIGN AND SETTING: A cross-sectional study in an urban area of Alcobaça, Brazil. METHODS: Men and women older than 60 years with (270) and without (184) abdominal obesity were included. Sociodemographic, health, and behavioral data were collected using validated questionnaires in Brazil. Descriptive and path analyses were performed (P < 0.05). RESULTS: The occurrence of falls was high in participants with abdominal obesity (33.0%). In both groups, a higher number of morbidities (β = 0.25, P < 0.001; β = 0.26, P = 0.002) was directly associated with a higher occurrence of falls. Among participants without abdominal obesity, a lower number of medications (β = -0.16; P = 0.04), a higher number of depressive symptoms (β = 0.15; P = 0.04), worse performance on the agility and dynamic balance tests (β = 0.37; P < 0.001), and lower functional disability for basic activities of daily living (β = -0.21; P = 0.006) were directly associated with the occurrence of falls. CONCLUSION: Adults older than 60 years with abdominal obesity have a higher prevalence of falls. Different factors were associated with the occurrence of falls in both groups.

ABSTRACT Objective: To analyze different anatomical sites in the abdominal region, in order to determine the positional parameter that identifies a higher level of visceral adipose tissue (VAT) and confers a greater cardiometabolic risk. Materials and methods: This is a methodological study in which VAT was evaluated by ultrasonography (USG) in three anatomical sites in the abdomen, while the abdominal circumference (AC) was measured using seven different protocols. Additionally, the glycemic and lipid profile, C-reactive protein, and the presence of systemic arterial hypertension were evaluated. Results: One hundred and six individuals with an average age of 42 (36.8-46.2) years were included. The evaluation of the calibration of the ultrasound procedure for the analysis of VAT by intra- and inter-evaluators showed high reproducibility. The pattern of abdominal fat distribution differed between sexes, with higher mean VAT in males (p < 0.05) and higher mean SAT (subcutaneous adipose tissue) in females (p < 0.005). In the abdominal scan applied to women, higher levels of VAT and lower levels of SAT were observed in the narrower waist region, between the iliac crest and the last rib (p < 0.001). In males, the profile of adipose disposition along the abdomen was uniform (p > 0.05). Correlations between VAT measured by USG and cardiometabolic parameters were relatively stronger in the upper abdomen (p < 0.05). Conclusion: Women accumulate more VAT in the narrower waist region, while men accumulate VAT uniformly across the abdomen. There was relative superiority in predicting cardiometabolic risk in the upper abdomen for both sexes.

ABSTRACT Introduction Combined training is more effective than an isolated modality in reducing cardiometabolic risk indicators. Objective To evaluate the effect of circuit training volume on anthropometric and biochemical risk indicators for cardiometabolic diseases in overweight women. Methods Thirty-two participants underwent 24 weeks of circuit training with free weights combined with aerobic exercise. The training volume during the 24 weeks was used to distribute the women into moderate-volume physical activity (MVA), low-volume physical activity (LVA) and control (CON) groups. Anthropometric indices (body mass, body mass index (BMI), waist circumference (WC), waist-hip ratio (WHR)), blood glucose, insulin, insulin resistance (HOMA-IR), total cholesterol (TC), triglycerides, HDL-c, and LDL-c were evaluated at the beginning of the program and after 12 and 24 weeks. Results There was no interaction between training volume and time for any of the variables studied, but the intervention time influenced body mass (p=0.013) and BMI (p=0.012), and there was a tendency for participation time to reduce body mass (p=0.063) and BMI (p=0.062) after six months of intervention. The volume of the physical activity affected HDL-c (p=0.037), being significant (p=0.030) in the comparison between the MVA and CON groups. Additionally, there was a downward trend in HDL-c after six months of intervention (p=0.073), with a smaller reduction observed in the MVA group, indicating a protective role of moderate physical activity in the reduction of this lipid fraction. The association between physical activity volume and participation time resulted in a clinical improvement in total cholesterol (χ2 = 5.453, p = 0.02), with a higher probability of reaching clinically adequate values in the MVA group (OR = 0.126; 95%CI 0.019 - 0.827). RESUMEN Introducción El entrenamiento combinado es más eficiente que la modalidad aislada en indicadores de riesgo cardiometabólico. Objetivo Evaluar el efecto del volumen de entrenamiento en circuito sobre indicadores antropométricos y bioquímicos con riesgo de enfermedades cardiometabólicas en mujeres con sobrepeso. Métodos Treinta y dos participantes se sometieron a 24 semanas de entrenamiento en circuito con pesos libres combinados con ejercicio aeróbico. El volumen de entrenamiento durante las 24 semanas se utilizó para distribuir a las mujeres en los grupos: actividad física de volumen moderado (AVM), actividad física de volumen bajo (AVB) y control (CON). Se evaluaron los índices antropométricos masa corporal, índice de masa corporal (IMC), circunferencia de la cintura (CC), relación cintura-cadera (RCC), glucemia, insulina, resistencia a la insulina (HOMA-IR), colesterol total (CT), triglicéridos, HDL-c y LDL-c al inicio del programa y después de las semanas 12 y 24. Resultados No hubo interacción entre el volumen y el tiempo de entrenamiento para ninguna de las variables estudiadas, pero el tiempo de intervención influyó en la masa corporal (p=0,013) y en el IMC (p=0,012), y el tiempo de participación tendió a reducir la masa corporal (p=0,063) y el IMC (p=0,062), después de seis meses de intervención. El volumen de actividad física afectó al HDL-c (p =0,037), siendo significativo (p=0,030) en la comparación entre AVM y CON. Además, hubo una tendencia a la reducción del HDL-c después de seis meses de intervención (p=0,073), observándose la menor reducción en AVM, lo que indica el papel protector de la actividad física de volumen moderado en la reducción de esta fracción lipídica. La actividad física y el tiempo de participación mostraron una mejora clínica en colesterol total (χ2 = 5,453, p = 0,02), con mayor probabilidad de alcanzar valores clínicamente adecuados de AVM (OR = 0,126; IC95% 0,019 - 0,827). Objetivos Avaliar o efeito do volume de treinamento em circuito sobre indicadores antropométricos e bioquímicos com risco de doenças cardiometabólicas em mulheres com excesso de peso. Métodos Trinta e duas participantes foram submetidas a 24 semanas de treinamento em circuito, com pesos livres combinados com exercício aeróbico. O volume de treinamento durante as 24 semanas foi utilizado para distribuir as mulheres nos grupos: atividade física de volume moderado (AVM), atividade física de baixo volume (AVB) e controle (CON). Os índices antropométricos massa corporal, índice de massa corporal (IMC), circunferência de cintura (CC), relação cintura-quadril (RCQ), glicemia, insulina, resistência à insulina (HOMA-IR), colesterol total (CT), triglicerídeos, HDL-c e LDL-c, foram avaliados no início do programa e depois de 12 e 24 semanas. Resultados Não houve interação entre o volume de treinamento e o tempo para nenhuma das variáveis estudadas, mas o tempo de intervenção influenciou a massa corporal (p = 0,013) e o IMC (p = 0,012), e o tempo de participação tendeu a reduzir a massa corporal (p = 0,063) e o IMC (p = 0,062), depois de seis meses de intervenção. O volume de atividade física afetou o HDL-c (p = 0,037), sendo significativo (p = 0,030) na comparação entre AVM e CON. Adicionalmente, verificou-se tendência de redução HDL-c depois seis meses de intervenção (p = 0,073), sendo a menor redução observada no AVM, que indica o papel protetor de atividade física de volume moderado na redução dessa fração lipídica. A associação entre o volume de atividade física e o tempo de participação mostrou melhora clínica do colesterol total (χ2= 5,453, p = 0,02), com maior probabilidade de atingir valores clinicamente adequados de AVM (OR = 0,126; IC de 95% 0,019 - 0,827). ABSTRACT

ObjectiveTo explore the effect of interaction of hypertension and abdominal obesity on morbidity of diabetes and to provide the scientific bases for preventing diabetes. MethodsA total of 11 914 people who took the physical examination in 2019 were examined with cluster sampling method for the interaction analysis. The data was collected by physical measurements and laboratory tests. SPSS 16.0 and interaction models were used for data analysis. ResultsIn the subjects there were 75.19% with abdominal obesity, 47.99% with hypertension, 16.26% with diabetes, and 39.42% had abdominal obesity plus hypertension. Multivariate logistic regression analysis showed, after adjusting gender and age, the abdominal obesity(OR=1.29, 95%CI:1.14‒1.46) and hypertension (OR=2.93, 95%CI: 2.63‒3.26) were related to high risk of diabetes. There was no multiplicative interaction effect between abdominal obesity and hypertension (OR=0.98,95%CI: 0.76‒1.27). The additive interaction was significant between abdominal obesity and hypertension with RERI=0.53(95%CI:0.09‒0.98), AP=0.14(95%CI:0.02‒0.26), and SI=1.24(95%CI:1.01‒1.52). ConclusionThere is a synergistic effect on morbidity of diabetes between abdominal obesity and hypertension. The improvement of weight and control of blood pressure could prevent significantly the diabetes.

Introducción : El Síndrome Metabólico (SM) es una entidad clínica que se manifiesta por obesidad central, dislipidemia, hiperglicemia y/o hipertensión arterial y se relaciona con el desarrollo de Diabetes Mellitus tipo 2 y enfermedades cardiovasculares. Objetivo : Determinar la prevalencia del SM en adolescentes escolarizados del Departamento Central en el 2021 y evaluar los factores de riesgo relacionados. Materiales y Métodos: Estudio descriptivo con componente analítico. Ingresaron 279 adolescentes de 10 a 16 años de 6 escuelas elegidas aleatoriamente. Se realizó una encuesta sobre estilos de vida, prácticas alimentarias, antecedentes familiares y perinatales. Se realizó medición de peso, talla, circunferencia de cintura (CC), glicemia, perfil lipídico y presión arterial. Para el diagnóstico de SM se utilizó los criterios de Cook. Aprobado por el Comité de Ética del IICS y del Hospital de Clínicas. Para el análisis se utilizaron tablas de frecuencias, mediana y Prueba del Chi cuadrado para razón de prevalencias (RP) con nivel de significancia p p90 36%. La prevalencia de SM fue del 10,7 %. La CC >p90 con RP de 31,4 (7,6-128), y la hipertrigliceridemia con RP de 46,8(14,8-147,7), fueron los componentes preponderantes de SM. El peso elevado al nacer y no realizar actividad física, fueron identificados como factores de riesgo significativo. Conclusiones: La prevalencia de SM fue 10,7%, se relacionó con mayor peso al nacimiento, obesidad y sedentarismo.


Introduction: Metabolic Syndrome (MS) is a clinical entity that is manifested by central obesity, dyslipidemia, hyperglycemia and/or arterial hypertension and is related to the development of Type 2 Diabetes Mellitus and cardiovascular diseases. Objective: To determine the prevalence of MS in adolescents enrolled in school in the Central Department in 2021 and to evaluate their related risk factors. Materials and Methods: This was a descriptive study with an analytical component. 279 adolescents aged 10 to 16 years old from 6 randomly chosen schools were included. A survey on lifestyles, eating practices, family and perinatal history was performed. Weight, height, waist circumference (WC), glycemia, lipid profile, and blood pressure were measured. Cook's criteria were used for the diagnosis of MS. The study was approved by the Ethics Committee of the IICS and the University Hospital. For data analysis, tables of frequencies, median and Chi-square test were used for the prevalence ratio (PR) with a significance level of p p90 in 36%. The prevalence of MS was 10.7%. WC >p90 with a PR of 31.4 (7.6-128), and hypertriglyceridemia with a PR of 46.8 (14.8-147.7), were the predominant components of MS. High birth weight and physical inactivity were identified as significant risk factors. Conclusions: The prevalence of MS was 10.7%, it was related to higher birth weight, obesity and sedentary lifestyle.

ABSTRACT

The Brazil, as well as the world, is in a transition process, with changes in the nutritional, epidemiological and lifestyle profiles. At the same time, a progressive increase in life expectancy and the growth of chronic non-communicable diseases (NCDs) have been observed in recent decades. Among them are cardiovascular diseases whose main risk factor is obesity. In this scenario, anthropometric indicators are essential for the early identification of obesity, especially obesity accumulated in the abdominal region. The conicity index is one of the recommended tools for identifying the distribution of body fat, as it is associated with cardiovascular and metabolic complications in the population, especially in individuals with NCDs. Therefore, the use of the anthropometric indicator as a screening tool both in primary care and in epidemiological studies is recommended for the early identification of abdominal obesity. O Brasil, bem como o mundo, está em processo de transição, com alterações no perfil nutricional, epidemiológico e de estilo de vida. Paralelamente, observa-se nas últimas décadas o aumento pro-gressivo da expectativa de vida e crescimento das doenças crônicas não transmissíveis (DCNT). En-tre elas, estão as doenças cardiovasculares que possuem como principal fator de risco a obesida-de. Nesse cenário, os indicadores antropométricos são essenciais para a identificação precoce da obesidade, principalmente daquela acumulada na região abdominal. O índice de conicidade é uma das ferramentas recomendadas para a identifica-ção da distribuição da gordura corporal, pois está associado as complicações cardiovasculares e metabólicas da população, principalmente dos in-divíduos com DCNT. Portanto, a utilização do indi-cador antropométrico como ferramenta de rastreio tanto na atenção primária quanto nos estudos epi-demiológicos é aconselhada para a identificação precoce da obesidade abdominal. Background: The prevalence of obesity is increasing in developing countries like India. This will place a great burden on the health infrastructure of the country. Early identification and intervention are the keys to managing this important problem. Since Asians tend to have more visceral body fat for any given body mass index (BMI), lower cut-off values have been suggested for different anthropometric parameters. Studies determining the correlation of Sagittal abdominal diameter (SAD) and SAD-height ratio (SADHtR) with total visceral fat volumes measured by computed tomography (CT) scan abdomen quantification are limited in the South Indian population. Aims and Objectives: This study was conducted with the objectives of measuring BMI, SAD, SADHtR, and comparing the correlation of the measured anthropometric parameters with visceral and subcutaneous abdominal fat volumes measured using CT scan abdomen in both the genders in South Indian population. Materials and Methods: This analytical cross-sectional study was done in a hospital setting with a sample size of 566 (276 men and 290 women-aged 30–70 years). Weight, height, and BMI were measured. SAD was measured electronically from a CT image at the L4–L5 level. SADHtR was derived. Total abdominal fat volumes, subcutaneous abdominal fat volumes, and visceral abdominal fat volumes were measured in cm3 from the dome of the liver to the pubic symphysis using software for CT-based quantification of fat volumes between ?190 and ?30 Hounsfield units threshold limits. The data were tabulated and analyzed to compare mean values between men and women and to find the correlation between the anthropometric parameters and visceral fat volume using Pearson’s correlation test. Results: The difference in mean values of BMI, SAD, and SADHtR between the sexes was statistically significant. SAD showed the highest correlation with visceral fat volume in both genders. BMI showed a higher correlation with subcutaneous adipose tissue volume than with visceral adipose tissue volume. Conclusion: Both SAD and SADHtR can be used as simple and reproducible anthropometric tools to identify the presence of increased visceral fat volume in both genders in our population.

Background: Non-communicable diseases (NCDs) kill 41 million people each year, equivalent to 71% of all deaths globally. Cardiovascular diseases account for most of the NCD deaths, followed by cancers, respiratory diseases, and diabetes. Detection, screening, and treatment of NCDs are the key components of response to NCDs. Aim and Objective: The objective of this work is to study the association of body mass index (BMI), abdominal obesity, and insulin resistance with hypertension and those with both diabetes and hypertension. Materials and Methods: A case–control study was done among 30–70 years old people who came to Government Medical College, Kozhikode between August 2014 and July 2015. They were divided into two study groups and one control group. Complete history, physical, and laboratory examination was done among them and the data were entered in a pro format. Results: The mean ages in each group were 57.73, 58.5, and 48.71, respectively, in each group. The male: female ratio was 1.37:1, 1.06:1, and 1:1 in each group. BMI, waist circumference, waist hip ratio, and waist-to-height ratio (WHtR) were increased progressively in patients with hypertension and those with both hypertension and diabetes mellitus, respectively. Insulin resistance was highest in patients with both hypertension and diabetes mellitus. Conclusion: Obesity is an important contributor to the development of Type 2 diabetes and hypertension. Among the parameters to measure obesity, WHtR is considered the supreme. Insulin resistance is found in hypertensives and those with diabetes and hypertension.

Resumo O objetivo do estudo foi analisar a variação temporal da ocorrência do excesso de peso e da obesidade abdominal em adolescentes de escolas públicas de uma capital do nordeste brasileiro. Trata-se de um estudo incluindo informações de três levantamentos transversais realizados nos anos de 2001, 2009 e 2017, com 2.496 escolares de 10 a 18 anos de ambos os sexos, residentes na cidade de Salvador, Bahia. O excesso de peso foi avaliado por meio do Índice de Massa Corpórea para idade (IMC/I) e obesidade abdominal foi avaliada pela circunferência da cintura e razão cintura-estatura. A regressão Pooled OLS (Ordinary Least Squares), de efeito constante foi utilizada para avaliar a variação dos indicadores antropométricos ao longo do período. Os resultados deste estudo indicam que a prevalência de excesso de peso cresceu 103% e os indicadores da obesidade abdominal cresceram 153% e 142%, respectivamente no período analisado. Aumento na média do Índice de Massa Corpórea e da Razão cintura estatura foi maior entre as adolescentes e da média do IMC e da circunferência da cintura entre aqueles de 14 a 18 anos de idade. Conclui-se que houve expressivo crescimento nas prevalências do excesso de peso e da obesidade abdominal no período de 2001 a 2017 entre os adolescentes da cidade de Salvador.


Abstract The scope of this study was to analyze the temporal variation in the incidence of overweight and abdominal obesity among adolescents from public schools in a capital in northeastern Brazil. It is a study including information from three cross-sectional surveys carried out in 2001, 2009 and 2017, with 2,496 students aged 10 to 18 years of both sexes living in the city of Salvador, Bahia. Overweight was assessed using the Body Mass Index for Age (BMI/A) and abdominal obesity was assessed using waist circumference and waist-to-height ratio. Pooled OLS (Ordinary Least Squares) regression, with constant effect, was used to assess the variation of anthropometric indicators over the period. The results of this study indicate that the prevalence of overweight increased by 103% and the indicators of abdominal obesity increased by 153% and 142%, respectively, in the period under analysis. The increase in the mean body mass index and waist-to-height ratio was greater among adolescents and in the mean BMI and waist circumference among those aged 14 to 18 years old. The conclusion reached was that there was a significant increase in the prevalence of overweight and abdominal obesity in the period 2001 to 2017 among adolescents in the city of Salvador.

Background: Depression is a commonly prevailing condition that goes undetected in clinical settings. Both abdominal obesity and periodontal disease have a bearing on mental health and have an impact on the quality of life. Objective: To assess the level of clinical depression in abdominally obese subjects with periodontal disease. Methods: Two hundred and ten subjects with a mean age of 37.45 ± 9.59 years (males = 117; females = 93) were grouped as per their abdominal obesity and periodontal status and assessed for their clinical depression levels (mental health) using the Centre for Epidemiologic Studies?Depression Scale (CES?D). Collected data were analysed. Results: The clinical depression score significantly varied in subjects with different periodontal status in both non?obese (F (2,102) = 113.66, P < 0.0001) and abdominally obese subjects (F (2,102) = 132.04, P < 0.001). Significantly higher depression score was demonstrated in healthy (P < 0.001), gingivitis (P < 0.001), and periodontitis (P < 0.001) groups in abdominally obese subjects. Conclusion: Clinical depression is significantly associated with abdominal obesity and periodontal disease in subjects with abdominal obesity and severe periodontal disease demonstrating higher depression scores

Abstract Introduction: Multiple definitions of metabolic syndrome (MS) are used in Peru, and there is currently no consensus on which definition should be used in clinical practice. Objectives: To compare cardiovascular disease (CVD) risk estimators, obtained using the ACC/AHA ASCVD Risk Calculator, and to assess their level of agreement with different definitions of MS in patients treated in Lima, Peru. Materials and methods: Analytical cross-sectional study. Medical records, collected through consecutive sampling, of 233 patients treated between October and December 2019 at the Hospital Nacional Hipólito Unanue, Lima, Peru, were reviewed. CVR risk was calculated using the online ACC/AHA ASCVD Risk Calculator, and the MS definitions of the WHO, NCEP-ATP III, IDF, AHA/NHLBI, JIS and ALAD were considered to compare CVD risk according to each definition. Agreement between the different MS definitions was calculated using the kappa coefficient based on the six levels of strength of agreement described by Landis and Koch. Results: The median CVD risk in patients with MS according to the definitions of the WHO, NCEP-ATP III, IDF, AHA/NHLBI, ALAD and JIS was 9.6 (3.9-20.35), 7.9 (3.1-18.6), 7.3 (3- 16.5), 7.8 (3-17.6), 7.1 (2.9-16.5), and 7.1 (3.1-16.5), respectively. The prevalence of MS according to JIS, IDF, ALAD, AHA/NHLBI, NCEP-ATP III and WHO definitions was 81.97%, 80.26%, 74.68%, 67.81%, 65.67%, and 51.14%, respectively. Agreement between the JIS criteria and the IDF, ALAD, NCEP-ATP III, and AHA/NHLBI criteria was 0.944, 0.787, 0.592, and 0.567, respectively, but it was 0.286 between the JIS criteria and the WHO criteria. Conclusions: In Peru, there are differences between CVD risk estimates depending on the MS definition used and considered in the present study, which may have an impact on the intensity of the therapeutic and preventive interventions performed in these patients.


Resumen Introducción. En Perú se usan múltiples definiciones de síndrome metabólico (SM); sin embargo, actualmente no hay un consenso sobre cuál definición usar en la práctica clínica. Objetivos. Comparar las estimaciones de riesgo cardiovascular (RCV), obtenidas mediante la calculadora de RCV de la ACC/AHA, y evaluar su grado de concordancia con diferentes definiciones de SM en pacientes atendidos en Lima, Perú. Materiales y métodos. Estudio transversal analítico. Se revisaron las historias clínicas, obtenidas por muestreo consecutivo, de 233 pacientes atendidos entre octubre y diciembre de 2019 en el Hospital Nacional Hipólito Unanue, Lima, Perú. El RCV se calculó mediante la calculadora virtual de RCV de la ACC/AHA y se consideraron las definiciones de SM de la OMS, NCEP-ATP III, IDF, AHA/NHLBI, JIS y ALAD para comparar el RCV según cada definición. La concordancia entre las distintas definiciones de SM se calculó mediante el coeficiente kappa con base en los seis niveles de fuerza de concordancia de Landis y Koch. Resultados. Las medianas de RCV en pacientes con SM según las definiciones de la OMS, NCEP-ATP III, IDF, AHA/NHLBI, ALAD y JIS fueron 9.6 (3.9-20.35), 7.9 (3.1-18.6), 7.3 (3-16.5), 7.8 (3-17.6), 7.1 (2.9-16.5) y 7.1 (3.1-16.5), respectivamente. La prevalencia de SM según las definiciones JIS, IDF, ALAD, AHA/NHLBI, NCEP-ATP III y OMS fue 81.97%, 80.26%, 74.68%, 67.81%, 65.67% y 51.14%, respectivamente. La concordancia entre las definiciones JIS e IDF, ALAD, NCEP-ATP III y AHA/NHLBI fue 0.944, 0.787, 0.592 y 0.567, respectivamente, pero entre la JIS y la OMS fue 0.286. Conclusiones. Existen diferencias entre las estimaciones de RCV según las distintas definiciones de SM usadas en Perú y consideradas en el presente estudio, lo que puede tener repercusiones en la intensidad de las intervenciones terapéuticas y preventivas realizadas en estos pacientes.

Abstract Background: Abdominal obesity has been associated with cardiovascular disease and may be modulated by dietary intake. The deep abdominal subcutaneous adipose tissue (dSAT) is a body fat compartment that can be estimated by using mathematical formulas. Objectives: To evaluate the association between dSAT estimated by the Deep-Abdominal-Adipose-Tissue (DAAT) index and dietary intake in patients with acute coronary syndrome (ACS). Methods: This is a cross-sectional study conducted with patients ≥ 18 years of age admitted to a tertiary hospital. Sociodemographic, clinical, and anthropometric (body weight [kg], height [m], waist, hip and neck circumferences [cm]) data were evaluated. A food frequency questionnaire was applied to identify each patient's nutrient intake. The DAAT index was calculated according to specific formulas for men and women. Possible association between food intake and the DAAT index was evaluated by multiple linear regression. The level of significance adopted was 0.05. Results: This study evaluated 138 patients, with a mean age of 61.2±10.8 years. Prevalence of obesity was 29.4% in men and 37.7% in women. Regarding waist circumference, 83% of the women showed values considered to be very high. The DAAT index was significantly higher in men when compared to woman (P < 0.0001) and proved to be positively correlated with proteins (r= 0.22, P= 0.01) and monounsaturated fatty acid (r= 0.18, P= 0.04) intake in the entire sample. After adjustment for sex, alcohol consumption, and levels of physical activity, the DAAT index was associated with the female sex (B= −129.84, P <0.001) and a sedentary lifestyle (B= 57.99, P <0.001). Conclusion: dSAT estimated by the DAAT index was not associated with dietary intake in patients with ACS.

ABSTRACT

Objective:To investigate the level of serum Omentin-1 in subjects with abdominal obesity, and to analyze the influencing factors of Omentin-1 and its relationship with body fat distribution, insulin resistance and metabolic parameters.Methods:A retrospective case-control study was conducted to analyze the clinical data of one hundred and fifty adults with abdominal obesity (BMI≥28 kg/m 2) who were randomly selected from Obesity Multidisciplinary Diagnosis and Treatment Center, Subei People's Hospital from Januray 2018 to December 2021. Meanwhile, 150 healthy adults were enrolled as a normal control group. Fasting serum Omentin-1 and glucose metabolism were measured, and homeostasis model assessment of insulin resistance index (HOMA-IR) was calculated. Body fat composition was measured by bioelectrical impedance analysis. The relationship between Omentin-1 and other variables were presented by the Pearson correlation coefficients. Stepwise regression model was used to analyze the influencing factors of Omentin-1. Results:The serum omentin-1 level of patients with abdominal obesity was (36.97±6.99) μg/L, that of normal control group was (72.35±6.09) μg/L. The difference between the two groups was statistically significant ( t=46.69, P<0.001). The body fat level of patients with abdominal obesity was (43.40±14.59) kg, that of normal control group was (13.78±4.13) kg. The difference between the two groups was statistically significant ( t=23.93, P<0.001). The fasting insulin of patients with abdominal obesity was 29.05 (22.01,34.60) pmol/L, that of normal control group was 127.90 (84.08,201.45) pmol/L. The difference between the two groups was statistically significant( Z=14.75, P<0.001). The HOMA-IR of patients with abdominal obesity was 0.87 (0.68,1.05), that of normal control group was 4.19 (2.77,7.31). The difference between the two groups was statistically significant ( Z=14.75, P<0.001). Pearson linear correlation analysis showed that serum omentin-1 levels were negatively correlated with BMI, waist circumference, waist-hip ratio (WHR), body fat, visceral Fat area (VFA), HOMA-IR, triglyceride, total cholesterol and low density lipoprotein cholesterol ( r=-0.825, -0.843, -0.756, -0.777, -0.835, -0.583, -0.429, -0.353, -0.503, -0.938, all P<0.001). Whereas, a significantly positive correlation was found between serum Omentin-1 levels and high density lipoprotein cholesterol ( r=0.528, P<0.001). Omentin-1 concentrations were not related to age or gender ( r=-0.093, -0.040; P=0.669, 0.489). In multiple linear stepwise regression analysis, only VFA remained significantly associated with Omentin-1 ( β=-0.026, t=-2.250, P=0.026). Conclusion:The level of serum omentin-1 in patients with abdominal obesity was significantly lower than that in normal subjects, and it was closely related to body fat distribution and insulin resistance. VFA is an independent influencing factor of serum omentin-1, which can be used as a biomarker of abdominal obesity related metabolic disorders.

ABSTRACT

The aim of the present study is to explore the association between abdominal obesity and chronic kidney disease (CKD) through a systematic review of published studies. Databases including Wanfang data, CNKI, VIP, CBM, Cochrane Library, PubMed, Web of science and Embase were searched up to July 2021 to collect longitudinal studies published in Chinese and English on the association between abdominal obesity and CKD. In order to avoid omission, reference lists of related articles were also checked manually. After literature selection, data were extracted and study quality was evaluated by the Newcastle-Ottawa scale. Statistical analysis of this study was conducted using Stata 11.0 software. Finally, five studies were included in this study. The results showed that abdominal obesity defined by waist circumference was not associated with CKD (OR=1.17,95% CI:0.93-1.48). According to the results of subgroup analyses, whether adjusted body mass index might be part of the reason of heterogeneity. Based on results of this study, the association between abdominal obesity defined by waist hip ratio and CKD, as well as the association between abdominal obesity and CKD in different genders, remains unknown. In conclusion, abdominal obesity might not be associated with incident CKD. However, more studies are needed in the future to explore this association.

Resumo Objetivou-se analisar a associação entre consumo de bebidas alcoólicas e adiposidade abdominal em adultos. Estudo transversal realizado com dados da linha de base do ELSA-Brasil (2008-2010). A amostra foi constituída por 15.065 servidores públicos de seis instituições de ensino e pesquisa (35 a 74 anos, ambos os sexos). Para identificar adiposidade central por meio das medidas de circunferência da cintura (CC) e relação cintura/quadril (RCQ), utilizou-se os pontos de corte preconizados pela Organização Mundial da Saúde. Para as análises estatísticas foi utilizado o teste qui-quadrado e modelos de regressão de Poisson ajustados por variáveis potencialmente confundidoras. Cerca de 40% da amostra apresentava CC e RCQ elevadas. A probabilidade de apresentar CC elevada foi 5% e 3% maior no grupo mais exposto de consumo de cerveja em homens e mulheres quando comparado ao grupo de referência [RP = 1,05 (IC 95% 1,02-1,08) e RP = 1,03 (IC 95% 1,00-1,07)]. Também foi encontrada maior probabilidade de apresentar RCQ elevada entre os maiores consumidores de cerveja [RP = 1,03 (IC 95% 1,00-1,07) em homens e RP=1,10 (IC 95% 1,04-1,15) em mulheres]. Maior número de doses/semana de bebida alcoólica aumentou a probabilidade de ocorrência de CC e RCQ elevadas, sendo mais importante a contribuição da cerveja.


Abstract The objective was to analyze the association between alcohol consumption and abdominal adiposity in adults. Cross-sectional study conducted at baseline data from ELSA-Brasil (2008- 2010). The sample consisted of 15,065 civil servants from six education and research institutions (35 to 74 years old, both sexes). To identify central adiposity by measuring waist circumference (WC) and waist-to-hip ratio (WHR), the cutoff points recommended by the World Health Organization were used. Poisson regression models adjusted for potentially confounding variables were tested. About 40% of the sample had elevated WC and WHR. The probability of having elevated WC was 5% and 3% higher in the most exposed group of beer consumption in men and women when compared to the reference group [PR= 1.05 (95% CI 1.02-1.08) and P R= 1.03 (95% CI 1.00-1.07)]. A higher probability of having a high WHR was also found among the highest beer consumers [PR = 1.03 (95% CI 1.00-1.07) in men and PR = 1.10 (95% CI 1.04-1.15) in women]. A greater number of doses/week of alcoholic drink increased the probability of occurrence of high WC and WHR, with the beer contribution being more important.

Abstract As obesity has reached epidemic proportions and given the current recognition of central adiposity as an important cardiometabolic risk factor, several researchers have focused on developing and validating predictive indexes and equations to evaluate Visceral Adipose Tissue (VAT). This study evaluates the applicability of the Visceral Adiposity Index (VAI) for predicting cardiometabolic risk in individuals treated in a hospital In the northeast region of Brazil. The VAT was evaluated by computed tomography (CT) and the VAI was calculated through specific equations for each gender. The sample involved adult and elderly patients of both genders followed up in a cardiology outpatient clinic. The following cardiometabolic parameters were collected: fasting glycemia, glycated hemoglobin, lipid profile, C-reactive protein (CRP) and uric acid. The simple linear regression was used to evaluate the explanatory power of the VAI in relation to the volume of VAT determined by CT. The predictive capacity of VAI in relation to the volume of VAT determined by CT was 25.8% (p=0.004) for males and 19.9% (p<0.001) for females. VAI correlated strongly with the triglyceride (TG) (p<0.001) and TG/high-density lipoprotein (HDL) ratio (p<0.001) and inversely correlated with HDL (p<0.001). Moreover, VAI showed low correlation with the following variables: abdominal circumference, total cholesterol, low density lipoprotein, fasting glycemia, and glycated hemoglobin (p<0.05). VAI was associated with variables considered as cardiometabolic risk factors, but exhibited a low predictive capacity regarding the volume of VAT determined by CT. Thus, caution is recommended in its use in Brazilian individuals.


Resumo Em razão de a obesidade ter alcançado proporções epidêmicas e dado ao atual reconhecimento da adiposidade central como um importante fator de risco cardiometabólico, diversos pesquisadores têm se dedicado em desenvolver e validar índices e equações preditivas para avaliar o Tecido Adiposo Visceral (TAV). Este estudo avaliou a aplicabilidade do Índice de Adiposidade Visceral (IAV) como preditor de risco cardiometabólico em indivíduos atendidos em um hospital no nordeste brasileiro. O TAV foi avaliado por tomografia computadorizada (TC) e o IAV foi calculado através de equações específicas para cada sexo. A amostra envolveu pacientes adultos e idosos de ambos os sexos acompanhados no ambulatório de cardiologia. Os seguintes parâmetros cardiometabólicos foram coletados: glicemia de jejum, hemoglobina glicada, perfil lipídico, proteína C-reativa e ácido úrico. Regressão linear simples foi empregada para avaliar o poder explicativo do IAV em relação ao volume de TAV determinado por TC. A capacidade preditiva do IAV em relação ao volume de TAV determinado pela TC foi de 25,8% (p=0,004) para o sexo masculino e 19,9% (p<0,001) para o sexo feminino. O IAV se correlacionou fortemente com as variáveis TG (r=0,916, p< 0,001) e TG/HDL (r=0,952, p<0,001) e inversamente com o HDL (r=-0,441, p<0,001), além disso, apresentou baixa correlação com as variáveis: circunferência abdominal, colesterol total, lipoproteína de baixa densidade, glicemia de jejum e hemoglobina glicada (p<0,05). O IAV associou-se com variáveis consideradas fatores de risco cardiometabólico, porém exibiu baixa capacidade preditiva em relação ao volume de TAV determinado pela TC, sendo recomendada cautela em sua utilização em indivíduos brasileiros.

Objective:To investigate the correlation between Helicobacter Pylori (HP) infection and abdominal obesity. Methods:A total of 1728 subjects who underwent routine health check-up and 13C or 14C urea breath test at the Health Management Center of Beijing Hospital from January 2018 to December 2018 were enrolled. The correlation between HP infection and metabolic syndrome (MS) components was analyzed. Results:There was no significant difference in HP infection rate between MS group and non-MS group [73.3%(360/491) vs. 68.7%(850/1 237), χ2=3.55, P=0.060]. The prevalence of abdominal obesity and elevated fasting blood glucose in HP positive subjects was significantly higher than that in HP negative subjects [71.7%(352/491) vs. 65.4%(809/1 237),χ2=6.36, P=0.042;80.7%(396/491) vs. 75.9%(939/1 237),χ2=4.50, P=0.034]. The non-obesity rate of HP positive subjects [28.1%(138/491)] was lower than that of HP negative subjects [34.3%(424/1 237), χ2=6.36, P=0.042). There were no significant differences in rates of hypertension, high triglyceride and low HDL-C, increased total cholesterol, increased LDL-C between HP-positive and HP-negative subjects (all P>0.05).There was no significant difference in HP positive rate between MS and non-MS groups [29.8%(360/1 210) vs. 25.3%(131/518), χ2=3.55, P=0.060]. The HP positive rate in subjects with abdominal obesity was higher than that with normal abdominal circumference [30.3%(352/1 161) vs. 24.5%(139/567), χ2=6.31, P=0.012], and the HP positive rate in subjects with elevated fasting blood glucose (FBG) was higher than that with normal FBG [29.7%(396/1 335) vs. 24.2%(95/393), χ2=4.50, P=0.034]. The HP infection rate was not associated with hypertension, hypertriglyceridemia and low HDL-C. Logistic regression analysis showed that abdominal obesity ( OR=1.327, P=0.016) was the risk factor of HP infection; and HP infection was the risk factor of abdominal obesity ( OR=1.277, P=0.043). Conclusion:HP infection is positively correlated with abdominal obesity and may be correlated with elevated fasting blood glucose, but not with other components of metabolic syndrome in health check-up population.

Objective:To analysis the correlation of eating speed with obesity.Methods:A total of 644 people aged 40-65 from Caihe Community in Hangzhou were enrolled to collect clinical and demographic data, undergo extensive physical examination and laboratory tests. Participants were divided into two groups according to their eating speed (non-fast and fast). Obesity-related parameters were compared between two groups. Multivariable logistic regression was conducted to explore the relationship between eating speed and obesity after adjusting confounders.Results:Body mass index, waist circumference, and visceral fat area were greater in the fast eating group than non-fast eating group(all P<0.01). After adjusting for age, gender, smoking, alcohol drinking, physical activity level per week, and principal food intake, logistic regression analysis showed that eating fast was correlated with abdominal obesity( OR=1.66, 95% CI 1.11-2.48, P=0.014) and visceral obesity( OR=1.65, 95% CI 1.14-2.39, P=0.007). After stratified by gender, in the group of men, eating fast was correlated with abdominal obesity( OR=2.04, 95% CI 1.07-4.04, P=0.032) and visceral obesity( OR=1.85, 95% CI 1.04-3.31, P=0.037); In the group of women, eating fast was correlated with overweight and obesity( OR=1.59, 95% CI 1.04-2.42, P=0.031). Conclusion:Eating fast is positively associated with obesity. Interventions for reducing eating speed may be effective for weight control.
